Electronic Medical Records: Who owns the work?

There is hardly anyone who is opposed to moving toward a digital strategy for medical records. It would allow for robust applications and services so we could understand the impact of multiple prescriptions, avoid adverse drug interactions, assess long-term therapies, reduce costs and headaches of entering new data every time you go to the doctor, and much, much more. It would like together providers in a way that allows them to actually communicate about a patient’s best interests instead of remaining isolated in the closed silos.

Despite legitimate concerns about privacy and security (which I do believe can be addressed with strong business processes, governance reform and good technology), there are only the questions of why it’s taking so long and who is in charge? Today’s New York Times takes a shot at it, and offers a compelling story about the $19 billion in federal dollars for a movement toward electronic medical records.

The real answer is that the institutional infrastructure of both government and private providers have little real incentive to aggressively move to an e-records system. The vendors sell expensive, proprietary hardware-centric systems that reinforce the silos of data and prevent easy access from competitors; doctors are generally technology-adverse; government doesn’t seem to realize it is often paying the bill and should care more and push harder. So the studies pile up, the reports come out, and the efforts to move toward a next generation system continue to plod along.

Local and state governments are being ruthlessly and viciously crushed by the cost of health care. It is destroying our way of life and sucking the resources away from education, affordable housing, environmental cleanup and more. One viable strategy to reduce costs is to implement comprehensive EMR (electronic medical records). But it takes leadership and bold commitment to push, prod and move the institution of government forward.

Obama deserves credit for raising the issue frequently although I don’t have enough information to know if they are making any real progress. It’s a top priority in the other Washington. The efforts in Washington State seem modest at best and seem paralyzed with talk or at best lack of communications. Or, perhaps, there is a bold effort underway and Washington is applying for a large piece of the $19 billion federal money. It’s possible and, as always, I love finding out that more is happening that meets the eye. As a member of the Energy, Technology & Communications Committee, I haven’t heard anything about it. I hope against hope there is a big push underway.

I don’t blame anyone in particular for my own lack of deep domain expertise and knowledge in this area….and naturally don’t assume I have a handle on everything we’re doing in Washington about federal competitive dollars in every area. I certainly don’t. But in a strange way that’s actually part of the point. Why don’t we have more communication about what the plan is, who owns it, when it’s due, how much we’re going for and what our success rate is for competitive grant dollars? Our strategy around federal competitive dollars–in virtually every area–is a black hole. We have little visibility into how we’re doing as a state from an objective point of view.

We have an image of ourselves as entrepreneurial, innovative and creative in the use of technology in government. That image is more a stereotype and cliche than reality. The imagine comes because we’re the home of Microsoft, Amazon, Real Networks and much more than it does because of local, county or state government’s next generation use of technology.

There’s $19 billion in federal money available for electronic medical records conversion strategies, tactics and programs alone. One, two or three years from now, I wonder whether we’ll look back on this and think, again, we missed the opportunity of this crisis. Again.

    All this inconvenience re: electronic medical records…
    Since computers—D E A T H for MEDICAL ERRORS has increased in the USA.
    Why? Because of AFFIRMATIVE ACTION recipients in the medical field!
    And because of foreigners automatically being hired because we have been overrun by ILLEGALS from all nations….one begets the others…
    The more Mexicans who run across the USA borders to birth just in time for auto-welfare citizenship from womb to tomb via sperm donor exponentials: the more Mexicans the USA TRAITORS from within hire. Most are hired by former recipients of AMNESTY, Visa overstays; birthing, etc.
    PLEASE NOTE: 2/3 of the population increase comes direct from Mexico.
    The USA paves over a land mass the size of the State of Delaware each year.
    RESULTS the demise of the hinterland, animal habitats; migratory paths and the results a lot of pollution; concrete and degredation…
    All to accomodate illegals and legals: but who is counting?
    Who benefits from the INVASION?
    Certainly, not US Troops nor their families whom have been systematically sacrificed on foreign soil each generation and now for OPEN BORDERS USA CRIMINAL ILLEGAL ALIEN INVADERS: drug runners(end up with green cards while US Border Patrol Agents guarding the USA are thrown in jail framed by TRAITORS within); heathen breeders via sperm donors of THE RACE receive auto welfare now EIGHT GENERATIONS with no contributions except NUMBERS USA; diseased; terrorism; special interest; foreign diplomats who pay nothing; even the Catholic Church insults parishiners by proclaiming they need Catholics from Latin America(to aid and abett more illegals as they bless their busses departing from MEXICO;
    The UN; whom we the people pay for their troops while ours are B E T R A Y E D as systematically sacrific ed on foreign soil for OPEN BORDERS AMNESTY continued as if USA citizenship is popcorn being REWARDED criminals who live to break USA Laws openly then are RE-warded. All goes on and onto take down One Nation Under GOD.
    ARABS Special Treatment from veil to call to workship over USA childrens diminished prayer which used to open class days to football games-no more: criminals or ARABS or Samolians demand a prayer room everywhere….
    ARABS buy sheep to slit their throats for their holidays….just like our USA soldiers.

    $20 BILLION to benefit whoM: who will control information; who will acess; how; who will know their medical records have been searched and for what, when, how, why?
    How about this:

    A GLOBAL SHOPPING CATALOGUE for the world to buy spare parts.
    Look at how many foreigners now arrive just in time to be the recipient of our D E A D
    How convenient: while we go deficit to bankruptcy…
    We are 4% of the worlds overpopulation: and the most debtor nation?
    How many debts of foreign nations have we the people had to forgive via our USA GOVERNMENT which many departments have DUAL CITIZENSHIP or LOBBYISTs certainly not fighting on foreign soil for us dba the USA?
    How many ILLEGALS and LEGALS have walked right in or are shipped like the recent over l00, 000 Palestinians or HAMAS? Auto welfare citizensHIP>>>>>
    What do our US TROOPS GET: how about filthy centers to wait for perhaps any medical care; denial of injuries or of mental illness due to having been re-sent to IRAQ E I G H T times and away from their families.     She is a photo op specialist who even runs to funerals or complain there aren’t enough BLACK NAMES for hurricanes! That is a rep who brought 20 members to the USA to provide care for a 47 yr old male & 27 yr old female from Nigeria who took fertility drugs: unemployed so we still foot the bill@! Wonder why we have bills, higher ins? EXAMPLE!
